Factors affecting the extraction versus non-extraction decision for the orthodontist
Abstract

The decision of whether or not to extract teeth in order to remedy malocclusion is a highly debated topic in orthodontics. When deciding whether or not to extract permanent teeth, clinicians must weigh many factors including: spacing or crowding, overjet, overbite, occlusal stability, temporomandibular dysfunction, periodontal health, facial esthetics, smile arc, and systemic health among others. One of the factors that makes extractions one of the most difficult treatments to diagnosis is the fact that a significant proportion of orthodontic cases fit in the “borderline” category – meaning extraction and non-extraction treatment plans can both be considered.
There is a body of research studying the trends of extraction and expansion throughout the history of the profession, but no significant attempts at creating an evidence-based way to make this decision. These two research projects were intended to move the research in such a direction.
The conclusions will be presented as well as clinical cases to illustrate this challenging but very interesting decision that involves the orthodontist of course but also his dental specialists colleagues.
